Art. 54 Evidence of training or of a course

The confirmation that provides evidence of training in accordance with Article 197 AniWO or of a course in accordance with Article 198 paragraph 2 AniWO must contain at least the following information:

a.
the logo or stamp with the name and address of the organiser;
b.
the surname, first name, date of birth, place or country of origin and place of residence of the person who has completed the course;
c.
the place and date of training and title of training;
d.
the place, date, name and signature of the person responsible for the training.

Art. 54 Attestation de participation à une formation ou à un cours

L’attestation de participation à une formation visée à l’art. 197 OPAn ou à un cours visé à l’art. 198, al. 2, OPAn doit contenir au moins les informations suivantes:

a.
le logo ou le timbre portant le nom et l’adresse de l’organisateur de la formation;
b.
le nom, le prénom, la date de naissance, le lieu ou le pays d’origine et le domicile de la personne qui a suivi le cours;
c.
le lieu, la date et l’intitulé de la formation;
d.
le lieu, la date, le nom et la signature de la personne responsable de la formation.
