¹ The statement of claim contains:

a.

the designation of the parties and their representatives, if any;

b.

the prayers for relief;

c.

a statement of the value in dispute;

d.

the allegations of fact;

e.

notice of the evidence offered for each allegation of fact;

f.

the date and signature.

² The following must be filed together with the statement of claim:

a.

a power of attorney where a party is represented;

b.

the authorisation to proceed or the declaration that conciliation is being waived, if applicable;

c.

the available physical records to be offered in evidence;

d.

a list of the evidence offered.

³ The statement of claim may include a statement of legal grounds.
