455.109.1 FDHA Ordinance of 5 September 2008 on Training in Animal Husbandry and Handling of Animals (Animal Welfare Training Ordinance, AWTO)
Art. 65 Prüfungsausweis
Wer die Prüfung bestanden hat, erhält von der für die Prüfung zuständigen Organisation einen Ausweis ausgestellt, der den Erfolg der absolvierten Prüfung festhält und mindestens folgende Angaben enthält:
- a.
- Logo mit Namen und Adresse der für die Prüfung zuständigen Organisatorin oder des für die Prüfung zuständigen Organisators;
- b.
- Name, Vorname, Geburtsdatum, Heimatort oder Herkunftsland und Wohnort der Kandidatin oder des Kandidaten;
- c.
- Prüfungsort und -datum;
- d.
- Bezeichnung der mit der Prüfung bestandenen Ausbildung;
- e.
- Ort, Datum, Name und Unterschrift der Prüfungsorganisatorin oder des Prüfungsorganisators.
Art. 65 Examination certificate
A person who has passed the examination receives a certificate from the organisation responsible for the examination, showing that the examination has been completed successfully and containing at least the following information:
- a.
- the logo with the name and address of the organiser responsible for the examination;
- b.
- the surname, first name, date of birth, place or country of origin and place of residence of the candidate;
- c.
- the place and date of examination;
- d.
- details of the training completed by the examination;
- e.
- the place, date, name and signature of the examination organiser.
